Los retos de las reformas sanitarias en el Sistema Nacional de Salud

espanolEste trabajo analiza las reformas que precisa el Sistema Nacional de Salud (SNS), las perspectivas desde las que abordarlas, y sus retos. Para ello, en primer lugar, toma como punto de partida y referentes normativos los valores de la sanidad publica y los principios de buen gobierno de lo publico; sus referentes empiricos son las caracteristicas positivas de los sistemas sanitarios similares al espanol. A continuacion, revisa los logros, caracteristicas y problemas del SNS, agrupando estos ultimos en problemas comunes a todos los sistemas de salud, y problemas especificos de la sanidad espanola. Para estos se propugnan dos bloques de reformas, de un lado reformas estructurales del sistema (ciudadania sanitaria, financiacion, sostenibilidad, politica de personal y gobierno sistemico del SNS), del otro, reformas para el buen gobierno y gestion de los Servicios de Salud de las CCAA y sus centros sanitarios. Finalmente se concluye vinculando los retos de las reformas del SNS a la calidad de las instituciones y de la democracia en Espana, y se destaca como problema la baja prioridad que tiene el SNS en la agenda politica espanola. EnglishThis paper is about the reforms needed by the Spanish National Health System (SNHS), the perspectives from which to address them, and their challenges. For this, two normative and one empirical referents are taken as the starting points. The normative ones are the values of a public health service, and the principles of good governance of the public sector; the empirical ones are the positive characteristics of other countries? health systems similar to the SNHS. Then, the achievements, characteristics and problems of the SNHS are reviewed, grouping problems into those common to all health systems and the ones specific of the SNHS. Reforms addressing the latter are presented in two groups: reforms deemed structural (true universal coverage, financing, sustainability, personnel policy and the systemic governance of SNHS), and reforms related to the good governance and management of the Autonomous Communities (Regions) Health Services and their health institutions. Finally, we conclude linking the challenges of reforming the SNHS to the quality of institutions and democracy in Spain; and identifying the relative low priority of the SNHS in the Spanish political agenda as one of its more conspicuous problems.
