Reformas y cambios organizativos en el Sistema Nacional de Salud español

espanolEste trabajo analiza y evalua los cambios organizativos recientes producidos en el Sistema Nacional de Salud espanol. Las jerarquias publicas centralizadas del INSALUD y otros servicios regionales de salud se estan orientando hacia modelos hibridos y relaciones contractuales a traves de dos instrumentos esencialmente: el contrato programa y la creacion de nuevas formas juridico-organizativas regidas por el derecho privado. Profundizar en estas reformas, generalizarlas y minimizar los potenciales efectos perversos que conllevan, implicaria cambios en el marco de la regulacion institucional y estrategias de superacion de la dependencia de senda en la que las organizaciones sanitarias del estadoespanol confinadas en la actualidad. EnglishThis study examines and evaluates the recent organizational changes that have taken place in the Spanish National Health System. The centralized public hierarchies of the INSALUD and other regional health services are being oriented towards hybrid models and contractual relations through two instruments in particular: the programme contract and the creation of new legal organizational formsgoverned by private law. Developing these reforms, generalizing them and minimizing the potential perverse effects they entail would involver changes in the framework of institutional regulation and strategies for overcoming the path dependence in which the Spanish state health organizations are confined at the present time.
